SWEEPSTAKES DATES
The Sweepstakes begins at 12:00:01 a.m. PT on January 10, 2011 and ends at 11:59:59 p.m. PT on February 6, 2011 (the “Sweepstakes Period”). All entries must be submitted and received within this time period. Any entries not received within this time period will be disqualified.
HOW TO ENTER
NO PURCHASE NECESSARY. Enter by completing and submitting the online entry form linked from www.ktla.com/taiwan. You must provide all information requested on the entry form marked as “required”, which includes but is not limited to name, address, and telephone number, and you must have a valid e-mail account in order for your entry to be eligible and qualified for the drawing. Incomplete entries will be disqualified and KTLA is not responsible for online entries that are lost, late, garbled, deleted or misdirected as a result of technical, internet or other online difficulties. In the event of a dispute regarding the identity of an entrant, the holder of the e-mail account will be deemed the person who submitted the entry. Limit one entry per person or e-mail address during the Sweepstakes Period. Subsequent entries from the same email address will be deleted. Multiple entries from the same person will be disqualified. Sponsors will make any and all final decisions in any and all issues related to this Sweepstakes, and their decisions shall be final and binding in all respects.
ELIGIBILITY
This Sweepstakes is void where prohibited by law. This Sweepstakes is open to legal United States citizens residing within the KTLA viewing area during the Sweepstakes Period, who are 18 years of age or older as of January 9, 2011. Employees of other media companies (including television and radio), as well as employees of Sponsors and their parent corporations, subsidiaries, or affiliates, and such employees’ immediate family or household members are not eligible to enter or win the Sweepstakes.
PRIZE AND WINNER SELECTION
On or about February 6, 2011, Sponsors will select one (1) Winner in a random drawing among all qualifying entries. The odds of being selected as a winner depend on the total number of entries received. If a selected winner is not eligible, the prize will be forfeited and awarded to an alternate winner selected by a random drawing from the remaining entries. Copies or other mechanical reproductions, facsimiles, entries containing technical or electronic reproductions, entries containing attached files are not eligible and will be rejected. In the event of a dispute regarding the identity of an entrant, the holder of the e-mail account will be deemed the person who submitted the entry. Entries become the exclusive property of Sponsors and will not be returned or acknowledged
One Winner will receive two (2) certificates from EVA Airways. Each certificate is valid only for exchange for an economy-class round-trip airplane ticket for one from Los Angeles International Airport to Taipei, Taiwan on EVA Airways. Approximate Retail Value of each certificate is $1,635.00.
THE PRIZE DOES NOT INCLUDE HOTEL OR ANY OTHER TRAVEL ACCOMMODATIONS OF ANY KIND.
The prize is subject to the following restrictions:
• Each certificate has no commercial value, cannot be resold, cannot be converted into cash and cannot be replaced for any reason. Winner is liable for all taxes applicable and must pay the taxes at the time certificates are redeemed for plane tickets. Accepted methods of payment for taxes due is at the discretion of EVA Airways and/or ticket issuing agency. Please contact your nearest EVA Airways ticketing office for tax information at either of the numbers listed herein.
• Winner must contact the EVA Airways reservations office to make the initial reservations by calling either 1-800-695-1188 or locally at 1-310-362-6600. Reservations must be made by February 7, 2012. Once reservations are made, the Winner must go to the EVA Airways offices at 200 North Sepulveda Blvd. Suite 1600, El Segundo, CA 90245. to redeem the certificates. No extension to the tickets’ period of validity will be granted. Redemption of airfare certificates will require presentation of acceptable federal identification. Certificate redemption and flight itinerary must be booked prior to departure.
• Prize is not applicable to Evergreen Club Program, EVASION Program or any other promotional programs.
• Any unused Prize elements will be forfeited. The actual value of the prize may vary depending on date and times of travel, airfare fluctuations and hotel charges at the time of travel. Therefore, the actual value of the prize awarded may be lower or higher at the time it is fulfilled. Any difference between actual value and stated ARV will not be awarded. Prize winner and his or her guests/travel companions will be required to travel on identical itineraries including the same departure date and return date. Prize winner and his or her travel companion will each be required to provide a valid government issued identification and/or passport at time of travel. If the prize winner elects to travel or partake in prize trip with no travel companion no additional compensation will be awarded to the prize winner. No changes will be made to travel details once any element(s) of the travel arrangements have been booked, except at Sponsors’ sole discretion. All prize-related travel arrangements will be administered by Sponsors’ authorized designee. All airline tickets issued in conjunction with prize are not eligible for upgrades, frequent flyer miles or any other promotional benefit. Sponsors will not replace any lost or stolen tickets, travel vouchers or certificates or similar items once they are in the prize winner’s possession, or in the possession of the prize winner’s travel companion. Prize travel is subject to capacity controls, availability, blackout dates and certain other restrictions. Travel must be roundtrip. Sponsors will determine airline and flight itinerary in their sole discretion.
• Once certificates are redeemed for plane tickets, the tickets are valid for 3 months after the date of issue. No extension to the tickets' period of validity will be granted.
• The tickets are subject to the following blackout periods when travel is prohibited:
The Prize is non-transferable, non-refundable, non-endorsable, non-reroutable, may not be combined with any other offer, and has no cash value. Prize substitution will not be allowed, except at the sole discretion of Sponsors. Sponsors reserve the right to substitute a prize of equal or greater value for the prize. Any unused Prize elements will be forfeited. Any and all expenses not set forth herein, including any ground travel expenses, bag-checking fees, taxes or incidentals, shall be the sole responsibility of the Winner.• Winner must contact the EVA Airways reservations office to make the initial reservations by calling either 1-800-695-1188 or locally at 1-310-362-6600. Prize is awarded “as is” with no warranty or guarantee, either express or implied, by Sponsors.
All international, federal, state, and local taxes associated with the prize, as well as all other costs or expenses associated with the prize, including, but not limited to, transportation costs, are the responsibility of the winner. Winner will receive a 1099 tax form reflecting the actual value of any prize that is $600 or more.
Winners will be notified by telephone and/or e-mail; if Sponsor is unable to contact a Winner by January 19, 2011, the Winner may be disqualified and prize may be forfeited, and a substitute winner may be selected.
The winners may be required to present valid identification and to sign a W-9 Form, a Statement of Eligibility and Liability and Publicity Release by February 28, 2011 in order to receive the prize. Failure to provide acceptable identification or timely execute and return any required forms may result in disqualification and forfeiture of the Prize.
GENERAL CONDITIONS
If for any reason, the Internet-related portion of this Sweepstakes is not capable of running as planned, including infection by computer virus, tampering, fraud, technical failure, or any other cause which corrupts or threatens the administration, security or integrity of the Sweepstakes, KTLA reserves the right, in its sole discretion, to terminate or suspend the Sweepstakes or any portion of the Sweepstakes. ANY ATTEMPT TO DAMAGE OR UNDERMINE THE FAIR AND LEGITIMATE OPERATION OF THIS SWEEPSTAKES WILL RESULT IN DISQUALIFICATION FROM THE SWEEPSTAKES. Information submitted in Internet entries, including entrant's name, address, e-mail address, etc., may be used by KTLA TV for marketing or promotional purposes.
By participating in this Sweepstakes, you agree to have your name, photograph, voice, biographical information, and likeness used in any and all media for promotional purposes relating to this Sweepstakes or future similar Sweepstakes without further compensation or notice, except as prohibited by law.
By participating in this Sweepstakes, you agree to be bound by these rules and the decisions of Sponsors, and you hereby forever and irrevocably release, indemnify and holds harmless each of the Sponsors and their officers, directors, employees, representatives, agents, shareholders, successors and assigns, from any liability, claims, demands, judgments, proceedings, causes of action, lawsuits, injuries, death, costs, expenses or damages, due to any injuries, damages or losses to any person (including but not limited to death) or property of any kind, arising from, resulting from, or in connection with, directly or indirectly, in whole or in part, from the Sweepstakes and any element or activity thereof, the entrant’s participation in, entry in, or in ability to enter the Sweepstakes, or any Sweepstakes-related activity, or the entrant’s acceptance, use, misuse, inability to use, possession, travel related to, or defect of any prize, or any violation of entrants privacy, publicity, personal and proprietary rights, or the collection, use and/or sharing of entrant’s personally identifiable information by Sponsor or its designees; any change in the prizing (or any components thereof) due to unavailability, business or creative considerations, or due to reasons beyond Sponsor’s control, any late, lost, stolen, mutilated, misdirected, illegible, delayed, garbled, corrupted, destroyed, incomplete, undeliverable or damaged entries, any wrongful, negligent, or unauthorized act or omission on the part of any of the Sweepstakes Entities, or any of their agents or employees, lost, stolen, damaged, delayed, or misdirected baggage, lost, late, stolen, misdirected, damaged or destroyed prizing (or any element thereof), and any typographical errors in these Official Rules or any Sweepstakes promotional materials.

A list of the winners’ names will be available after the winners have been verified and the prizes have been awarded.
